Has my patient achieved adequate myocardial reperfusion?
نویسنده
چکیده
For two decades, the goal of reperfusion has been restoration of normal epicardial blood flow.1–4 It has, however, become increasingly apparent that not all TIMI grade 3 flow is created equally, and attention has shifted downstream to the restoration of normal myocardial perfusion to fully optimize clinical outcomes.3,4 The goal of this Clinician Update is to provide clinicians with a better understanding of myocardial perfusion in terms of its importance, assessment, and treatment in clinical practice.

Pathophysiology of Ischemia/Reperfusion-induced Myocardial Injury: What We Have Learned From Preconditioning and Postconditioning?
Organ damage after reperfusion of previously viable ischemic tissues is defined as ischemia/reperfusion injury. The pathophysiology of ischemia/reperfusion injury involves cellular effect of ischemia, reactive oxygen species and inflammatory cascade. Protection against ischemia/reperfusion injury may be achieved by preconditioning or postconditioning. In this review, we discuss basic mechan...
متن کاملCardiac magnetic resonance imaging and endothelin-1: a step forward in the detection of microvascular obstruction.
In recent years, cardiac magnetic resonance has become an indispensable clinical and research tool widely used for the assessment of cardiac function and infarct size in patients following an acute myocardial infarction (MI). Left ventricular size and function are well known and important predictors of adverse remodeling and cardiac death after an ischemic event. However, the presence of myocar...
متن کاملPrimary percutaneous coronary intervention: devices to prevent no-reflow phenomenon
Primary percutaneous coronary intervention, where available, has become the therapy of choice in myocardial reperfusion during ST-elevation myocardial infarction. However, in a significant proportion of patients, owing to a high thrombus burden, myocardial perfusion is not fully achieved in the epicardial vessel. This phenomenon has been attributed predominantly to the distal embolization of th...
متن کاملHypothermia and percutaneous coronary intervention during acute myocardial infarction
Hypothermia has proven to be one of the most potent and consistent adjunctive therapies for infarct size reduction in numerous preclinical studies, when administered prior to reperfusion. The mechanisms leading to protection are multifactorial. Early clinical trials failed to demonstrate a decrease in infarct size, likely due to the difficulty in achieving adequate cooling prior to reperfusion....
متن کاملRole of myocardial contrast echocardiography in the clinical evaluation of acute myocardial infarction.
T he ultimate goal of treatment in acute myocardial infarction (AMI) is to salvage as much myocardium as possible with the least possible risk to the patient. In the immediate aftermath of reperfusion therapy, the clinician must determine whether the infarct related artery (IRA) is patent and if so whether successful myocardial reperfusion has been achieved. Addressing these questions expeditio...
